Hey everyone, hoping someone has been seen or know about something similar.

My insurance is requesting that a doctor approve a 2-week short-term medical leave but my situation involves multiple providers,

I saw a first doctor  after a severe panic attack who prescribed me something I had a bad reaction to and I ended up in the ER which was quite traumatizing where le urgentologue gave me a note but asked me to check with le GAP, I then followed up with a third doctor via the GAP who again spoke about my hormonal problems not my mental health problems . Now I'm confused about who is actually responsible for filling out the insurance form ? the ER doctor? The GAP doctor? or original doctor? the insurance doesn't seem to understand that I don't have one single family doctor managing everything 

Has anyone dealt with this kind of difficult care situation in Québec and figured out who "owns" the dossier for insurance purposes? Any advice appreciated 🙏
