What advice would you give a 25 HCP golfer?

This is a shot I find myself in often. I could easily shave 6+ strokes if I could master getting on the green and not sending it down the hill. Is a flop the only answer? Closest flag very little green to work with before it’s running down hill. Right now I either leave it short on the hill or i pop it up and come off too hot.
