What could be the reason that my players don't position themselves well and leave a lot of space to the opponent's players?

Also, when I lose the ball they don't press on their own .. only the ones I control.

I use mostly 4-2-2-2 or 4-1-3-2 formations and I switch between Possession and Quick Counter. My overall rating is 3100.

Theoritically .. What I could be missing?
