Private social worker to help manage care of disabled brother?

My brother in law has been suffering from pretty debilitating disability since his stroke, and it's clear that being bounced between SNF and rehab and home and back to the hospital isn't working. Even worse is that he isn't really capable of advocating for himself anymore, though staying local to his partner is important to him.

I live across the country and have traveled a few times earlier in this situation to help, but it's clear I don't know enough of the San Diego area (or CA for that matter) and the next steps and available resources to really get this set up appropriately. Trying to get help from the social worker at the SNF is like pulling teeth and honestly rarely answers the phone or returns messages anyway.

Is there some sort of private case management available that knows the area? Or maybe you know a place that would provide live in care for someone disabled in their 50s?
