I originally herniated last July, 9 months ago. I did PT, and I thought I had gotten better as the tingling and pain stopped and I graduated. The tingling came back in mid March and I ended up seeing the Ortho again. Just got my 2nd MRI report and I'm seeing myself never being active again.
FINDINGS:
Spine Numbering: For purposes of this dictation, it is assumed that
there are 5 non-rib-bearing, lumbar-type vertebrae, and the most caudal
fully segmented lumbar vertebra is labeled L5.
Alignment: Normal
Vertebral Bodies: Normal in height.
Marrow Signal: Expected
Intervertebral Discs: L3-L4 disc space height loss.
Cauda Equina: Normal
Extraspinal Soft Tissues: Normal
Individual Levels:
T12-L1: Normal
L1-L2: Normal
L2-L3: Normal
L3-L4: Small circumferential disc bulge with superimposed left
subarticular disc extrusion with inferior migration, facet arthropathy,
mild ligamentum flavum thickening causing mild canal stenosis with left
subarticular zone narrowing and displacement of the descending left L4
nerve root, mild bilateral neural foraminal stenosis.
L4-L5: Small circumferential disc bulge, facet arthropathy, ligamentum
flavum thickening causing mild canal stenosis, mild bilateral neural
foraminal stenosis.
L5-S1: Small circumferential disc bulge, facet arthropathy without
significant canal stenosis, mild bilateral neural foraminal stenosis
with abutment of the bilateral exiting L5 nerve roots.
IMPRESSION:
- L3-L4 left subarticular disc extrusion displacing the descending
left L4 nerve root.
- Mild multilevel degenerative changes without high-grade canal or
neural foraminal stenosis.
