So one of the quests was "Encounter Twisted Sprout 3 times", on one of my runs on the first day of the toon of the week, I encountered Twisted Sprout 2 or 3 times, but apparently the game only registered it as 1 encounter, I thought it only applied when Twisted Sprout appeared on different runs and not different floors, so I tried again and again to get him to spawn. Now today on my recent run, I did encounter him again but unfortunately died, the game didn't register it and it was still 1/3, I thought that I was supposed to survive the floor to make the encounter register, but I don't know anymore. I didn't know how to report this supposed "bug" so I went here in Reddit instead to hopefully know why those encounters didn't register.
