i need some advice

Hi guys,
I would like to built a saas for physical therapist, in order to simplify their process of data monitoring and patient scheduling. It has to be super complete. I don’t know how to start, because no-code are not so secure for sanitary data, i don’t want to risk.

I recently built and launched a project, an Italian platform, currently live and testing in my local area.
What I built:
It's a digital hub designed to connect people with local brick-and-mortar businesses. It essentially functions as a rich local directory (restaurants, shopping, health & beauty, professionals) but with active marketing features built-in. Through the platform, users can discover local commercial activities, find active Offers & Events, download Coupons, and participate in a Fidelity/Loyalty program.
For businesses, it’s a way to get local digital visibility and drive actual foot traffic to their physical locations.

My current challenge:
The platform is fully functional, but I’m currently at a crossroads on how to properly monetize it. Since this relies heavily on small to medium local businesses, I want a model that feels fair to them but is also sustainable for me to grow the platform.
What business models would you recommend exploring for a local directory/coupon platform?
Here are a few ideas I'm tossing around, but I’d love your input:
• Freemium Listings: Basic visibility is free, but businesses pay a monthly subscription for premium features (like creating custom coupons, events, or using the fidelity system).
• Featured Placements: Charging a flat fee for "Top of the page" or "Highlighted" spots (Sponsored listings).
• Pay-per-Performance: Charging businesses a small fee only when a user downloads or redeems a coupon.
• Setup/Onboarding Fee: Charging a one-time fee to set up a professional page and digital loyalty card for older businesses that aren't tech-savvy.
Have any of you built or worked with something similar? What monetization strategies actually work for local retail and restaurants nowadays?
