I've run into several patients over the years who are able to answer the questions and are "a&ox4" but as I continue to talk or work with them, theres just something "off" about them where you can tell they are not all the way there. What are some ways to document or describe this? Some of the obvious ones I will describe the situation such as "requires increased time to/for xyz", but is there a general phrase that I could be using that is appropriate?
