Condom slipped off at the end, pregnancy risk?

Had sex May 4. First 2 rounds were fully protected. Third round too, but when I pulled out, the condom was left inside her and not on my dick anymore, with the open end still sticking out outside.
I’m not sure if it slipped off only at the end or if I accidentally kept going without noticing for a short time.

I didn’t finish and wasn’t even close. I also peed and washed between rounds. Her app showed it was the last fertile/ovulation day, so now I’m stressed. Realistically, how low is the pregnancy risk here?

Concern About Possible Pregnancy Risk After Condom Slip

Five days ago, I had sex with my girlfriend three times within less than two hours. The first two times were definitely with a condom. The third time also started with a condom, but at the end, when I pulled out, the condom had slipped off and stayed inside her, and we removed it afterward. I’m not completely sure whether there was any moment during the third time when penetration happened without the condom, because I couldn’t really feel it.

Between each round, I washed my penis well and urinated. During the third time, there was no ejaculation inside her — I finished later by myself in the bathroom.

My concern is: in the worst-case scenario, if there was brief penetration without the condom and only possible pre-ejaculate (“pre-cum”), how likely would pregnancy be? Is this considered a very low risk situation, or is there a real reason to worry?
