T2 here. Been on metformin + basal — fasting started going too low so my doc reduced the basal. Now post-meal is what's killing me — keeps hitting 200s. Just started rapid a few weeks ago.

Basal is fixed but I'm supposed to adjust rapid before each meal and I honestly have no clue. My ICR feels like a wild guess. I keep undershooting because I'm scared of going low, then watch my sugar climb anyway after I eat.

Eating out is the worst — I have no idea what's actually in the food, so I'm basically guessing every time. I end up under-dosing because I'm afraid of going low, and then I just spike anyway.

CGM is too expensive to wear all the time, so I only use it a couple weeks every few months. Most of the time it's finger pricks, but by then I've already missed the spike.

For those who've been on mealtime insulin for a while — how did you actually figure this out? Was it just trial and error over time? Anything that actually helped?

Especially curious how people manage this without CGM.

Honestly feels like I'm guessing every meal.
