Father (61) scheduled for Prostatectomy next week for Gleason 6. He’s hesitant and I’m worried we’re rushing. Advice?
Hey everyone,
My dad is scheduled for a radical prostatectomy next week, but he’s having major second thoughts. He’s only doing it because my mom is terrified, but he’s worried about losing his quality of life (he’s still sexually active).
The Stats:
- Gleason: 3+3 (Grade Group 1)
- Biopsy: Only 1 out of 16 cores positive.
- Tumor: 2mm (very small).
- Spread: None. No nerve or vascular involvement.

- PSA Test Results
11.12.2025 6.25 ng/mL
14.01.2026 4.95 ng/mL (It went down after taking some medication for prostatitis/ enlarged prostate)
He’s 61 and otherwise healthy. From what I’m reading, Gleason 6 is often managed with Active Surveillance, yet the surgeon went straight to the OR schedule.
Questions:
- Has anyone with a similar "low-risk" profile regretted jumping straight to surgery?
- Is Active Surveillance a viable path for someone his age with such a small tumor?
We feel like we're rushing into a life-altering decision. Would love some perspective.
TL;DR: Dad has low-volume Gleason 6. Surgery is in 7 days, but he’s hesitant. Is Active Surveillance the better move here?
