I’ve been a bit concerned lately that my diagnosis of insulin resistance may not be accurate. I originally started seeing an endo because I noticed I was gaining weight if I wasn’t on a calorie deficit (5’8” around 190lbs female). I have an appointment set with my doctor but I’m hoping someone here has something helpful.

I get most of my nutrition through a g-tube (think ensure but much more nutritionally dense) but I can still manage around 700 calories a day by mouth. Admittedly, I indulge in empty calories a few times a week but it doesn’t feel frequent enough to cause ir.

Exercise is a separate battle. I have fibromyalgia and can’t move as well as I used to, but I still make an effort to move as much as I can without going into a flare (I use a visible band for this). I’m definitely more sedentary than I’d like to be, but again—it doesn’t seem like enough to cause insulin resistance.

My original labs showed fasting insulin at 25 and fasting glucose at 100. My repeat after being on metformin for around a month and a half is fasting insulin at 16.4 and fasting glucose at 92. HOMA-IR is at 3.73.

I have a glucometer at home, and my fasting levels are constantly jumping around. I’ll be at a normal level for several days in a row and suddenly it spikes high again.

My triglycerides and cholesterol are slightly high as well—which my primary told me is likely genetics—but I’m questioning if it’s something deeper.

I brought up the thought of possibly having LADA to my psych, thinking she would tell me how that’s not the case, but instead starting telling me all the ways that it’s possible.

If anyone knows of anything that can help this situation I’d appreciate it, I’ve been spiraling so much about which direction my life is going to take next.
