23M – nocturnal awakenings with urge to void, 2-year history

  • 2-year history of fragmented sleep with awakenings every 1.5–3h
  • Each awakening associated with urge to urinate; unable to fall back asleep unless voiding
  • Frequently low urine volume (sometimes minimal output)
  • Rapid return to sleep after voiding

Associated features:

  • ~95% of awakenings accompanied by nocturnal penile tumescence
  • Reports ability to suppress urge, but experiences conditioned inability to resume sleep without voiding
  • First sleep cycle consolidated; subsequent sleep fragmented
  • Describes “physically sleepy, mentally alert” upon awakening

Onset:

  • Began during aggressive weight cut (120kg → 90kg in ~2.5 months)
  • Concurrent stimulant use (caffeine, L-carnitine)

Current status:

  • 3–7 awakenings/night
  • 7–8h time in bed, non-restorative sleep
  • Denies choking/gasping; occasional dry mouth, nasal congestion

Interventions attempted:

  • Fluid restriction 2–4h pre-bed → no reduction in awakenings
  • Melatonin → no effect
  • Multiple dietary approaches → no significant change

Labs (Feb 2025):

  • Glucose normal (85 mg/dL)
  • Urinalysis normal (no infection, no glycosuria, SG 1.021)
  • Creatinine 1.31 mg/dL, eGFR 76 (G2), urea mildly elevated
  • Lipids: LDL elevated (141 mg/dL), total cholesterol borderline
  • CRP low
  • Thyroid (FT3/FT4) normal
  • Testosterone total mid-low; free testosterone low-normal
