Young adult female cat with outter ear infection not getting better after medication for 3 days

Hi all! I'm posting this on my sister's behalf. Her cat is a neutered 1.5yo female Siamese cat, and she has been some ear infection issues for a while. They are in nothwestern China, and she would like a second opinon since the cat is not getting better after getting medication for 3 days.

From what i understand, it started with the kitty scratching her ears, which resulted in some fur loss. My sister contected the vet, and they said it was itchy from scabbing. So she did iodine cleanup daily. But the cat's ear was looking reded and more inflammed so she brought her in. It's diagonsed as otitis externa, and the cat was tested postivie for cocci (? I'm not too sure which bateria) and Malassezia. The vet prescribed Oridermy and Amoxicillin, but my sister said the kitty isn't getting any better after 3 days of treatment, and she's a bit worried. Here is the picutre of the ear my sister sent me today. Does the cat need something else to fight with the infection?

A bit clinical history. She got the cat when she was maybe 3, 4 month old from a breeder; (1) there's not animal rescue shelter in her city 😞 (2) the cat was looking quite rough among the others. So the cat was postivie for FHV-1 and was sick for a bit but got better after treatment. I think she got sick for a couple of times too but pretty mild and not as serious as this time. My sister is just a bit skeptical about the vet service back home and would appreciate a lot for any advice!
