Hi everyone! I’m an undergraduate student working on a population health class project focused on understanding health experiences and barriers in the Harlem community.

My group created a short, anonymous survey to learn more about access to care, health education, screening, transportation, and other challenges that may affect people’s health locally. The survey should take about 5–10 minutes to complete.

Your responses are completely anonymous, and the information will only be used for our class project. Participation is voluntary, and you can skip any question you do not want to answer.
