Won't heal

Please any advice is greatly appreciated.
My dog has an open wound that heals and it opens up again in the groin area.
I have taken him to the vet 4 different time, 3 times with the same vet, and once with a new one. He has completed 4 rounds of antibiotics pills and ointment. The vet checked for foreign objects that might be stuck inside, parasites and did a blood work and found nothing. The wound fully heals and I make sure that he doesn't drag his groin on the grass or the ground. Today, noticed blood on his bed and saw that he was bleeding from it again. I took him to the vet and asked if he needed stitches, but he said that it wasn't necessary and gave him another round of antibiotics.
I don't know what to do anymore tbh. He's been dealing with this for months and the treatment works but I don't know what to do to keep it form opening again. I have tried hydrocolloid bandages when we walk but it just opens so easily.
