4yo cat vomiting and diarrhea – All tests came back clear but no improvement.

My cat is 14 years old. He is an indoor cat who has always been healthy. For the past few months, he has been having irregular episodes of diarrhea and vomiting. This week, things took a turn for the worse, and I rushed him to the emergency vet.

He stayed there for two nights and underwent a series of tests, including full blood work and an abdominal ultrasound. The results came back normal. The vets suggested he might just have a sensitive stomach, but I’m still very concerned. I have a follow-up appointment in 4 days, but the situation isn’t improving.

He is currently on medication + Smecta (dioctahedral smectite), but he just vomited several times in a row. Despite this, he remains alert, plays with me, and doesn't seem lethargic. Regarding his diet, I feed him veterinary gastrointestinal dry food; I avoid wet food as it seems to make it worse. There are no plants or toxic products in my home.

I’m struggling to understand how all the tests can be "fine" while he is still so sick. Are there any specific tests I should ask for (pancreatitis fPLI test, food allergies, IBD markers, etc.)? Any advice on how to manage this until my appointment in 4 days?
