Has anyone gotta waivers approved for the following:

1. Braces

to help with TMJ

1. - I’m willing to remove them

2. Unknown mass from childhood on my cheek. - have recently tried to get the notes from this and was told they were destroyed.

3. Skin cancer cells removed prior to age 25

5. ADHD

6. Anxiety - situational haven’t needed treatment since 2020.
