37 y.o/male/5’8/165lbs - occasional drinker / non-smoker
Went into ER October 2025 for palpitations, was kept for a few hours, given fluids and sent home. Got set up with a 3 day holter monitor which showed avg heart rate at 122 bpm and 76% of the time tachycardic.
Did an echo in December, summary:
- Normal LV size with moderately reduced global systolic function. LVEF 35%.
- Normal RV size and function.
- No significant valvular dysfunction.
Got sent to a cardiologist who ordered an MRI and entered into GDMT, began medications Dec. 19th
another 3 day holter ordered in early Feb. that showed avg hr 103 and 63% of time tachycardic.
Medication is 5mg Bisoprolol, 40mg Valsartan, 10mg dapagliflozin, 25mg eplerenone
MRI was done late April and came back,
INTERPRETATION SUMMARY: 1. Mildly dilated LV with moderately reduced ejection fraction (EF 38%) and global hypokinesia. 2. Normal RV size with reduced ejection fraction and global hypokinesia. 3. No valvular heart disease. 4. No late gadolinium enhancement. 5. T1 times and ECV: Normal myocardial native T1 times and ECV values suggesting the absence of interstitial fibrosis, myocardial infiltration and inflammatory process. 6. Incidental cardiac findings: None
I am still waiting to hear back from the cardiologist who I haven’t heard from since my December appointment.
I am wondering what to expect from here?
Is a 3% EF increase in ~4 months from the medication working or the MRI being able to measure more accurately?
