I’m running into a confusing issue while scheduling Step 2 and would really appreciate some guidance.
My passport only has a given name + father’s name — there is no surname/last name listed. However, the FSMB application requires both a first name and a mandatory last name.
During my Step 1 process, I accidentally entered a full name (given name + father’s name as surname) in the NotaryCam appointment, which caused an objection and pushed me into a name correction process.
Now I’m unsure how to proceed for Step 2.
My questions:
What should I enter in the “last name” field if my passport doesn’t have one?
Should I repeat my given name, use my father’s name, or something else?
Has anyone else with a similar passport format successfully scheduled without issues?
I want to avoid another mismatch or delay, especially with verification.
Would really appreciate advice from anyone who’s dealt with this situation 🙏
