Post visit report

Hi. I recently lost my dog unexpectedly after she had been battling an infection. I know the veterinary team was doing everything they could to help her, and I truly don’t blame them for the outcome. I’ve always appreciated the staff at this office and have trusted them with my family’s pets for years.
After her passing, I requested copies of her records because I had accidentally thrown away the original bloodwork report. While reviewing the notes, I noticed statements saying that emergency care had been recommended on two visits and that I declined. I also saw documentation stating that a follow-up appointment would be scheduled in a few days.
What’s troubling me is that I do not remember emergency care being recommended at any point during our conversation. If it had been presented to me that way, I absolutely would have taken her immediately following the conversation. I also was never told to schedule a follow-up appointment, nor was one arranged.
I understand emotions and grief may be making this feel heavier, but these discrepancies in the records are difficult for me to process. I’m not looking to place blame, I just want clarity and to better understand what happened. Is this something worth discussing with the office directly? Is it better to just find a new vet office?
