radical reduction and nipple sensation

hi everyone,

i just had my consult with dr. paul costas in concord mass. he was excellent and very kind; i obviously don’t have a post op review of him yet but he made me feel very secure and gave me realistic info grounded in his extensive experience doing reductions and top surgeries.

i am a 34i, and i was hoping to get a radical reduction to get to somewhere around a 34b. i’m genderqueer and use she/they pronouns, so in addition to this being a medically necessary surgery, it’d be secondarily affirming to go as small as possible while still maintaining a “breast” shape. that’s why i wanted a radical reduction, which he has lots of experience in.

during my consult, he said he could get me to any size i wanted, but to get any smaller than a large c cup i’d be looking at FNG and loss of nipple sensation. he said some people regain regular skin sensation after FNG, but not erotic sensation.

i don’t know what to do. might be tmi, but erotic nipple sensation is a big part of my pleasure in the bedroom. it’s not necessary for me to have a good time, but it’s really something i was hoping i wouldn’t lose entirely. i’m really scared i won’t be happy either way i decide; if i don’t go as small as i wanted to go, i could regret wasting the one surgery covered by insurance just to end up unhappy with how big they still are. if i go as small as i wanted to, i could regret the loss of erotic sensation and risk it affecting my romantic life.

my question to all of you is: do you have any advice on which one i should prioritize? do you have experience with losing erotic sensation after FNG and how it has affected your romantic life or pleasure in the bedroom? do you regret going one way or another, prioritizing desired size or retaining sensation? are there any success stories with FNG from those who highly valued erotic sensation prior to their reduction, but who don’t regret losing it?

For context, I’m a midsize person (5’3, 200ish lbs) who identifies as genderqueer and uses she/they pronouns. I got a referral for a medically necessary breast reduction from my PCP for back pain, not GAC, but I’m kind of using it as a win win where I can go smaller with a radical reduction to feel the gender affirmation alongside getting my physical issues with my chest resolved. I’d be aiming to go from a 34i to somewhere around a 34a or 34b, so I need someone who will listen to how small I want to go (I know FNG is likely inevitable with that size goal). Aesthetically, I’m not looking for chest masculinization, but rather just very small breasts. I’m under a time crunch because I won’t have access to my folks’ insurance after I turn 26 in the beginning of next year, so any advice is super appreciated!

hi! i’m 25yo and use she/they pronouns. i identify as genderqueer.

i am a 34I and am looking for someone willing to listen to me about wanting to go down to a A / B cup range at least. the reason is that 1) i want to have kids eventually and i REALLY don’t want them to grow back with those hormonal changes, which i’ve heard is very common, and 2) i want to be able to have “slightly there” breasts. in my dream results, i’d love if there was no or minimal skin to skin contact, like my chest wouldn’t overhang and touch the skin of the abdomen at all.

i know a nipple graft might be necessary to get me to that size range, but i’d love to find a surgeon who has experience in radical reductions / GAC to see if they can get me to that size without compromising the nipples. i don’t care about being able to breast feed, i just like having sensation and would be more happy with the results if i didn’t have to lose feeling in that area forever. however, i’m willing to have it happen if it gets me to a size where i don’t have to have a second reduction after having kids if that makes sense. basically i’m not looking for someone who will err on the side of larger to retain nipple sensation, but instead someone who will try their best to maintain nipple sensation but is going to do what they need to do to get me down to a small size.

is there anyone in the boston or boston-adjacent area who is like that? preferably a tier 1 MGB Health Plan provider?
