Hi all - what are the mandatory/support documents I need to submit in OINP to declare/prove spouse relationship?

I heard that I should fill up IMM5409E form and attach that in OINP application. Though, my question is that this form is from IRCC and we are doing application to OINP. so, how does it make sense to attach IRCC form in oinp application?

How do I find the payment of receipt for my work permit extension? I need to attach that with my PNP application?

I am seeing two option under GC key

1. Wp ext
2. Submission confirmation
3. Confirmation of Online Application Transaction

I can download first two pdf but not the last one. Can anybody help please?

I got invited for OINP FSW and in my work history, I have got 4 jobs in total. Out of which, first 3 jobs are from Walmart and Majorel. Whereas last one is IT (current job). This current job made me eligible for OINP. So my question is , Do i need the paystub / reference letter / statements of all the jobs that I have done so far? Or do I only need all these documents for just current IT job which made me eligible for the OINP FSW?
