My Daughter is in kindergarten and has not spoke the whole school year she is verbal but won't speak at school teacher said she has not said one word they did send home a parent request for evaluation acknowledgement form and it was an option for me to to fill out but I have not yet hoping that she would speak and I really don't know what the Acknowledgement form request is ??
