My mother (58F, HER2+ breast cancer) recently had brain metastases treated and now doctors suspect possible leptomeningeal disease (LMD). MRI findings are not 100% definitive.

Symptoms: increasing drowsiness/sleepiness without clear improvement despite tumor shrinkage.

Echo: normal EF (55–60%), no heart failure symptoms.

Doctors are recommending Whole Brain Radiation Therapy (WBRT) starting tomorrow.

My concern:

- What if this is NOT LMD?

- Is it reasonable to proceed with WBRT based on suspicion?

- Has anyone faced a similar situation where diagnosis wasn’t certain?

Looking for experiences from patients or caregivers who had to decide under uncertainty.

MRI findings (relevant to possible leptomeningeal disease):

- Initial report: “No leptomeningeal disease identified.”

- Addendum (re-evaluation):

- Ill-defined dural-based cerebellar lesions with folial distortion

- Linear leptomeningeal enhancement along lesion interfaces and adjacent folia

- Findings are concerning for leptomeningeal involvement, but appear less conspicuous compared to prior imaging

- Technical limitation:

- Significant motion artifact affecting posterior fossa evaluation

- Impression:

- Findings concerning for leptomeningeal involvement in posterior fossa (less prominent than prior study, possibly affected by technical factors)

- Recommendation for CSF cytology / flow cytometry correlation

- Additional findings:

- Stable brain metastases with slight decrease in size

- Multiple new acute/subacute infarcts (strokes)

- No intracranial hemorrhage or major mass effect
