Curious to stories of the hardest blue test challenge you’ve pulled off (phishing, pretexting, etc) and how you pulled it off?

I’m assuming it’s normal for dat centers to make sure they’re up to date against threat actors? what’s the hardest part of getting through security stopping at the gate?

If someone were to try and compromise a health record system, or workstation, how hard would it be?
