Upper back tightness and breathing issues

Looking for advice because even with physiotherapy this still doesn’t fully go away.

I’ve had upper back tightness around the thoracic spine for a long time now, mostly around the shoulder blades/ribs. It’s not really pain, more constant tension and sometimes a shortness of breath feeling like I can’t fully breathe deeply.

Doctors already ruled out heart problems and I’m currently seeing a physiotherapist for mobility exercises. The strange part is that mobility/stretch exercises or becoming more active again often make it worse for a few days afterwards. If I avoid exercises, the symptoms are still there but less intense.

I used to get this during full body workouts too, especially with overhead press and bent over rows, but it always went away before. Now it seems to stay around much longer.

No numbness or tingling, mostly just stiffness/tension. It’s making it difficult to get comfortable exercising again.

Has anyone experienced something similar? What helped you recover from it?
