Hi guys, I could really do with some advice. I was diagnosed about 20 years ago privately, then was looked after by CAMHS for most of my childhood (I think they might have even re-assessed me but can't remember and don't have the paperwork). After going to uni I moved around a few gp surgeries before settling where I am now.

I've been prescribed meds (originally concerta and now elvanse) since I was 7. When I moved to my current surgery my GP pointed out I dont have an up to date shared care agreement and they dont have any records for my ADHD besides the fact I am prescribed Elvanse. They referred me to local mental health services but I never heard back from them.

So that's the background, onto the issue.

This Monday 27th i got a notification from my GP surgery saying they won't be prescribing under shared care agreements from the 1st of April (26 days ago). I went onto my NHS app and saw they removed Elvanse from my prescriptions on there. They also sent me a form to fill out about what I want to do, the deadline is the 4th of May to fill that out.

Honestly I really don't know what to do. I got diagnosed so long ago I dont really know who it was by and don't know much about these shared care agreements or what right to choose is or anything. Im firing off subject access requests to gather as much documents and things as I can since my GP seems to have nothing on their system.

But what do I do? I don't really have more than 20 days of meds left and I really do need them for my job and for driving. Any help would be much appreciated, I am panicking quite a lot. I can't believe it's legal for them to kick me off this after 20 years with 0 notice
