Hello buddies,

Male, 30s. Wanted to see if anyone here had something similar.

I had an old anal fissure and they suspected there might also be a small subfissural fistula/infected cavity. I went in for EUA expecting possible fistula surgery, but during surgery they ended up doing a fissurectomy + curettage/cleaning of the area, not a classic fistula operation.

Recovery was kind of mixed. I improved, but never fully healed.

Main symptoms now:

  • ongoing drainage/moisture from the wound
  • occasional light bleeding
  • BM themselves are usually manageable, but still uncomfortable
  • the real problem is afterwards: internal anal pain/pressure/burning for 1 to 6 hours, depending on the day
  • if I don’t poop, I feel almost normal most of the day

At around 4-5 months they ordered an MRI because they were worried about a fistula/abscess, but it came back clean: no active fistula tract (good news!), no collection, just post-surgical/scar changes.

Now my CRS wants to do a second surgery to remove scar tissue and partially close the wound.

Has anyone had this kind of non-healing fissure surgery situation, with drainage and post-BM pain but no active fistula on MRI? Did a second surgery help? I’m really confused!
