Cause: A de Havilland Canada DHC-2 Beaver operated by Mountain Air Service collided with a Taquan Air DHC-3 Turbine Otter over George Inlet. Consequence: The failure to see and avoid other aircraft resulted in the disintegration of the DHC-2, killing all 5 people on board, and severe damage to the DHC-3, resulting in 1 death and several injuries after a forced landing.
