PA of 8 years with background in ER and interventional pain management. Recently transitioned in to orthopedic surgery. The ER I worked at had a peds specific ER, and I didn’t cover that side so pediatric fractures are newer and somewhat challenging to me. I know the Salter Harris classification system, but was hoping for some help here. New ortho attending diagnosed this kiddo with a Salter-Harris 2 fracture. I’m having a hard time seeing that.

I see that the physis isn’t well defined, symmetric, or fully opaque here, so I get that there’s SOMETHING going on. I just can’t for the life of me find a fracture line that’s proximal to (above) the physis or extends from the physis to the metaphysis.

Can someone help me out with these films, and maybe trace the fracture line so I can understand how this is a Salter Harris 2? Thanks in advance!
