Rib pain when sleeping on the side, anyone else?

Heya,

I just wonder if anyone else has this. It's been happening in every long fast or rolling fasts I've been doing (and I've done a lot over the past 14 years).

Basically after a few days of fasting (or rolling fasts) it becomes pretty painful to sleep on either side. It feels like my ribs are being crushed inside, I can't explain it better. Like I'm being squeezed. I can only sleep on my stomach or back, I wake up with pain if I move on my side in my sleep.

My guess as always been that it was the fast weight loss : I have a large frame, a large ribcage, and perhaps it's moving based on how much fat is around and inside it (my ribcage really seems to get smaller when I'm at a lower weight based on photos). Meaning during a fast weight loss it's really getting "squeezed" inside, and the process is getting too fast if I add pressure by sleeping on the side. The pain completely disappears once my weight gets stable again.

I never worried about it, it's just annoying. But I've also never seen anyone talk about it either on weight loss subs or fasting subs. I can't be the only one ??

Using a long fast to stop or reduce GLP1 (also I have MCAS)

TL;DR

For health reasons I'll probably need to be around 0,2mg wegovy for years. Right now I'm on 1mg of wegovy and it's rough on my digestion of fats. I need to stay keto for health reasons so that's a problem. I'm looking if anyone has experience using long fasts to lower glp1 suddenly without hunger rebound.

I'm a very experienced faster. Over the past 14 years it's been the only way for me to regulate my health and weight. Until I discovered carnivore, because the lion diet (ruminant meat and fat only) also gives me the same result. But I can't stand to do it long term. So for the past few years it's been an alternance of keto, lion, fasting and eating whatever.

When I say "experienced" I mean I've done dozens of multi day fasts, multiple of which were over 21 days, one 40 day fast in 2019, ADF for 6month+, multiple types of rolling fasts (rolling 48, 72 etc for months...) I ate 20:4 for years as well, sometimes strict OMAD for months.

My weight is all over the place because when I'm not fasting or eating lion I'm constantly ravenous (fasting didn't cause that. My biggest weight now is still 20kgs lighter than before I began fasting 14 years ago). Even when eating OMAD. I finally gave up and asked to try a GLP1 this winter. And to my surprise the first injection helped... my sleeping issues that I'd had since I was a child.

That's how I was diagnosed with MCAS (Mast cell activation syndrome).

Turns out MCAS messes up hunger and satiety (it basically destroys leptine and GLP1 and frequently causes very fast weight gain), as well as sleep (when my melatonin goes up I get a pseudo-allergic reaction that makes me super awake...), and it causes a bunch of other health problems (like lipedema, which I have). Fasting and lion diet are well known to make MCAS asymptomatic, that's why it's helped regulate me so much over the years. And why unlike most people I'm used to sleeping much better when I'm fasting that when I'm not. And GLP1 can help dramatically with MCAS.

So now that I know what I have I'm taking some medicines : PEA (a fatty acid that's lacking in MCAS people), it made a big difference for my sleep. Some antihistamines especially an H2 before going to sleep. Methylen Blue because covid gave me PEM and MB keeps it asymptomatic (maybe even could help cure it? In theory it helps the cells restore their mitochondria with time). A bunch of supplements (turns out all my long term favorites are known to help with MCAS, like ginkgo biloba..) And GLP1.

I know that MCAS is made worst by excess fat and by scar tissue (and lipedema is basically a mix of both). So my goal is to reduce those as much as possible with a mix of fasting, massages, wearing compressions, lion diet and GLP1. And then to keep my weight down while reducing slowly my various medications to see which ones need to be kept long term or not. Probably fasting here and there to help wean of them.

So I've begun this plan and for now it's working. But at this point I'm taking Wegovy at 1mg and it's too much. I don't like it. I feel like it's messing up my digestion too much. Especially my digestion of fat, which is a problem when eating keto and even worst for lion diet. But if I get the dosage down suddenly I'm afraid of the backlash for my satiety and inflammation and to go back into a weight gain cycle (inflammation causing ravenous appetite etc).

I'm thinking a long fast would probably help but I'm curious if anyone has experience and advice on the subject. Or if there are studies about it (I haven't found any). Basically if I were to do a 21 days fast or so, should I keep injecting during the fast but lower the dose every week? Or should I stop completely and get back on a low dose after the fast is over? Long term I'd like to be on 0,2mg or so as that seems to be enough for MCAS effects and as it is a popular dosage in the keto and carnivore spaces.

For now my plan would be to inject before going into the fast to make it as easy as possible, then not inject at all until I'm done fasting + doing one week of lion-diet refeeding, then going straight into 0,2mg per week.

Also if you have any advice or experience with MCAS messing with satiety and how to help with it I'll take it as well ! I'm feeling a lot less desperate now that I know WHAT I'm fighting, but any new information is welcome !
