Using a brush pen someone left with a bunch of Crayola markers in 'free' box.

I've been creating a few above-ground dungeons, and PCs have had to make climbing checks to reach secret spots or to navigate tricky areas (glassrock, or say a bare or mossy area). So far no one has died, but there have been a few harrowing close calls, which is all the better.

So that got me wondering, has anyone had a PC actually die while climbing? Like: they fail a climb check, and then they fail the saving throw to catch themselves on a hand hold, and splat.

(To be fair, I suppose what often might happen is some prick attacks you while you're climbing, and then you die.)
