Garfield the stray in MD (help!)

This is Garfield. He is an outdoor cat that has recently been living in my parents garage as he is getting too old for outside life. My parents are planning to move out in June and Im worried about him because he has been vocalizing a lot more and doesn't eat much and threw up last night. I think he may be sick and I dont know what to do bc he is terrified of humans (Ive know him for 5+ years and still have never pet him :( Do I call my local Vet and ask for someone to drive to the garage to check on him? Does anyone know of any services for community cats in MD that can help? Thanks to anyone who replies!
