Where is the dividing line between MCAS and histamine intolerance?

My doctor declined to give me a diagnosis of MCAS because my symptoms stop short of my airways literally closing up. She gave me a histamine intolerance diagnosis instead. I have yet to find a clear diagnostic guideline for where one stops and the other ends. It’s definitely not a “I’m fine as long as I take DAO supplements” type of person

Is there a firm line between the two or is it basically the discretion of the doctor?

I’ve been chasing a chronic illness for years. Probably autoimmune based on ANA, but we just found out my adrenocorticotropic hormone is low (likely related) and I’m being referred to endocrinology.

Now, typically I just go with whoever the referral gets sent to but I’m done wasting my time with doctors who dismiss me or aren’t compassionate. I’m choosing my own and asking for the referral to be sent over.

ANA was 1:640 homogenous and speckled

Red patches on both cheeks that flare when everything else does and then goes away.

Joint pain, body wide pain, malaise & low grade fever

In and of itself, lupus sounds like a reasonable conclusion. But coincidentally another doctor ran cortisol and acth (thinking they were high) and the acth was low and cortisol was at the very low end of the normal range.

Not asking for a diagnosis, but does this ring any bells for anyone? Of all the things, LOW cortisol and acth were the last thing I expected. I’m stumped 🤷‍♀️
