Not overally painful - is surgery worth it?

Been battling with Hemorrhoids for 20 years due to chronic diarrhoea. Currently not bleeding, they are causing problems like leakage, itching and still having bad bowel despite FODMAP and GF diet.

Would the surgery be worth it?
