I had received a provisional diagnosis of ADHD by my University Clinic two years back. Am I bound to disclose this in my MedXpress form?

Since it wasn't a full fledged diagnosis, it was only written that if the actual diagnostic test suite would be performed, I would likely get a positive result. If I don't have to disclose, then I would rather not, and prevent the hassle of going in for fast track or standard track.

I have started flight training, wanted to get through the formalities before putting down any money.

For more context, I don't have debilitating ADHD or anything. I am aware that I do forget some things sometimes. But I still function fine, don't face any serious issues in my day to day.

I am also looking flying as a part time thing. I have a full time job and don't look to start a career as a commercial pilot in the foreseeable future.

Edit:
I have never taken ADHD meds. I have never received any medical treatment for ADHD at all. Just normal run if the mill therapy for day to day shit we all go through.
