Help approaching my boss for leave at half pay

Cross posting from AUSCORP - there might be more understanding of ADHD and RSD in here.

A bit of a complex situation. I've been through an investigation at work. I am still employed, the other person resigned (it was a resign-or-we'll fire you kind of situation). The investigation was drawn out over multiple years, and I'm thoroughly exhausted. I have mental health challenges (anxiety, depression) and I've just been diagnosed with ADHD (with rejection sensitivity). The rejection sensitivity makes this all makes sense - I am hyper sensitive to rejection and experience it as trauma, which is what was my perception of what was happening at work.

This is all very embarassing as I'm a highly paid manager and working professional. I want to take 12 months off to work on my mental health, get some therapy, develop some better coping skills. I can afford this without too much struggle.

I have enough leave (annual leave, long service leave) to take leave at half pay for all of 26/27. My boss isn't obliged to approve that leave. I'm not covered by an EBA, and I am paid above the upper threshold for FWC unfair dismissal.

How can I approach this? My boss knows I have mental health concerns, and is generally supportive, but that doesn't mean he will approve this kind of leave. Would getting a sick leave certificate help me out?

I don't want to go down the work cover path, as the investigation isn't worth the additional strain on my mental health. I also don't want to approach this the wrong way and get managed out. Likewise, I can't keep going the direction I was going, as I'm in survival mode.

Why don't other states have conditional approval like NSW? Is conditional approval something that has worked effectively? With PTT rates in QLD as high as they are, it might be administratively simpler for QLD to just adopt the NSW approach.
