Pineconed honey gourami

There is clearly something I am doing wrong during my water changes. I used to have 2 honey gouramis. The first one pineconed the same day I did a the water change and 2 days later it died. That was 2 months ago. I did a water change yesterday and the second one started pineconing after the water change. Right now he is breathing agitated and sitting at the bottom of the tank on the gravel doing the same stuff the first was doing before it died. I have had the gouramis for almost a year along with 10 cardinal tetras, 6 ember tetras, 4 elegant corys, 6 kuhli loaches and 1 peacock gudgeon. I have had German blue rams in the past that have also pineconed right after me doing a water change and die the days shortly after.

I am keeping up with weekly water changes. I didn’t clean the filter as to want to keep beneficial bacteria. Is it because I am gravel vacuuming each time I do a water change? I have gravel substrate and 10 Amazon Swords as my choice of plants. The tank is a 55 gallon and has been set up for over a year now and is fully cycled. I am using API Stress+Coat to dechlorinate the water.
