I am caregiver for my father, 65M Pancan. Responded well to chemo and sbrt till now.
However his whipple will be complicated due to SMA/SMV involvement.

We are based out of India, and would like any help, on how to secure a video consult at a top pancreatic center, to evaluate the case and scans better.

Already tried MD anderson and MSK and they refused. Pancan network was also not very helpful.

Know really little about US healthcare to coordinate this, can someone please help with any ideas?

I feel, there is a good chance he can be helped, if we find the right doctors

My father 65M was diagnosed in Aug’25. Since then, with six rounds of chemo and 10 days of SBRT, his tumor has shrunk to sub 2 cm (started of at 8X5 cm)

Recent EUS testing shows no lymph nodes/ metastasis. Previous CT scans have showed SMA SMV involvement due to which doctors suggested a divestment based whipple. He is doing perfectly ok with normal weight, and appetite

Given the complications with whipple, and difficult recovery, is that the only option? Can such a small tumor be operated non invasively or maybe other minor surgical procedures to remove it?

Has anyone had similar experience with a shrinking small remnant tumor in pancan, what treatment course did you take?

My Father was diagnosed with Stage 3 in Aug 25

Ca 19 reduced to 54 post chemo (initially 10k+) which ended on 21st Feb

SBRT with MRLINAC happened between 13th to 26th Mar.

Now 1 month post Radiation, his CA19 stands at 204.

For people who had experience with such a regimen, could the rise be radiation related? Have you observed such increase after radiation?
