Tonight I loaded my part normally like I’ve always done but I forgot to finish tighten the part on the final operation. I use a hand drill to tight it but forgot to final tighten it with a T- handle. As it was on the final tool cutting the tabs out, the part wasn’t secure so it cut the tabs all the way off, the part came off and went into the conveyor. The conveyor alarm out and the machine stop from there. I inspected to see if there was any other damage but there wasn’t (I’m lucky it was the final operation and the part costed like 50-100$). I wasn’t rushing when loading, I just simply forgot to because after I hand drill tighten the bolts, I immediately loaded op 1 which took me 20-25min to load because of multiple steps it takes, and I just didn’t take the time to double check. I just feel bad and stupid about it because it was a mistake that could’ve been avoided if I double-triple checked my bolts.
