Has anyone here failed an OA?

I recently took an OA and failed due to a technical issue (MyEducator Tab disappeared before I could submit) The proctor worked hard to relaunch the session while the timer ran and huge chunk of my exam was missing.

Now the instructor says I need to complete a success plan before retaking or reach out to Assessment Services to review the recording of the exam.

What would be faster? Assessment services or going the success plan route?
