This survey is confidential and voluntary. While voluntary, the success of this effort depends on receiving completed surveys from as many employees and supervisors as possible. The survey is being sent to a sample of Government employees and supervisors.
The survey will remain open through May 22nd and should take approximately 30-60 minutes to complete. All responses will remain confidential.
