How do you handle formal PO received via email? Do you manually enter and validate everything into your accounting tool?

Happening quite often nowadays, do you guys also face this problem?

Question for the recruiters who use ATS (like Bullhorn, etc), have you ever paid to source a candidate externally (LinkedIn, job boards) only to find they were already in your database? Has dirty/duplicate candidate data ever cost you money or wasted your time? I am researching this problem - drop a comment if this sounds familiar.
