Does anyone have any experience with a New York LMFT supervisor filling out Form 4B attestations in a way that was not how you thought it should be filled out?

My supervisor is someone who is very matter of fact and filled out the paperwork with a long range of dates and a large sum of clinical hours.

Has anyone seen if they only look at the supervisor attestations? The numbers don't match what I put on page 1. My supervisor included more clinical hours. Has anyone had their full LMFT license being delayed by the paperwork not being filled out to match?

Also, separate has anyone had hours rejected as an LMHC supervised a LMFT candidate? My 1,500 hours are separated between multiple supervisors over the years.
