Hello, I recently sent my CV to this company via their HR email. I got a response to fill out a google form BUT I am having doubts because they're asking for my License Number?

Has anybody had any experience with Home Health Care Placements Inc.? I would like to know. Hindi naman sa gusto i-assume yung worst PERO I've already had an experience with a holistic clinic before that used my license number to procure like their IVs and stuff (quit and resolved na to), SO I am VERY skeptical when it's asked.
