Advice on Next Steps

I want to firstly say I’m so appreciative of this community, I know I don’t post often on here but y'all have been nothing but helpful!
My husband (28) and I (25) are on Cycle 12 of TTC. And have recently done many of the beginning test to try and figure out why there is no baby yet. I’d like to post all of our findings here and get advice on what you all think would be a good next step. For some reason I can’t post photos here but I’ll try my best to explain.
Semen Analysis (Jan 2026)
• This was done through our PCP referral so he interpreted the results, PCP said everything was normal.
• On the report the only thing that said “Abnormal” was Viscosity.
• OBGYN also glanced at them and said they looked normal but admitted she was not the expert in SA’s
PCP appt (Jan 2026)
• Labs were drawn for me and my husband
• Labs included STD testing
• PCPs main concern was my thyroid and A1C- both came back normal.
• my insulin was elevated- 25
HSG (April 2026)
• Normal HSG
• Also my HSG experience was positive overall, I’d rank it a 6/10 in pain during the procedure.
Labs Day 3 and Day 21 (April 2026)
• HbA1c- 5.4
• TSH- 1.870
• PRL/prolactin- 10.0
• Estradiol- 39.6
• Testosterone- 1.7
• FSH- 4.6
• LH- 6.7
• Progesterone- 7.3
Other Notes:
• Normal/ Predictable 28 day cycles
• Was on hormonal birth control from age 15-24
• My husband and I eat well and exercise regularly, I am overweight but working on it.
• I just began tracking my BBT this month, but throughout the process my partner and I have has regular (every other day) intercourse during my fertile window.

My OBGYN reports my FSH and LH indicate possible PCOS. When my PCP took my labs back in Jan 26 everything was normal I did have elevated insulin levels. OBGYN says that next steps would be 1-3 cycles of Clomid if that is unsuccessful she will referral us to a RE. OBGYN recommended following up with a Urologist to double check the SA results.
My Questions:
• Is seeking a PCOS diagnosis worth it in this process?
• If we are not pregnant this cycle should we begin Clomid next cycle?
• Can I seek a PCOS diagnosis & begin Clomid?
• Is a follow up with a Urologist necessary?
• Is there something we are missing?
