Hello, do I need to email my hats and ladders certificate to anyone?

Hello all. I was informed of my acceptance into SYEP this week and so I uploaded most of my documents. They're approved except for one tax form. It's incomplete and it doesn't have a link where I can access to fill it out, so I contacted my provider just today, but I'm worried it's too late because all forms are due today at 11:59PM.

Im considering to email the form to my worksite monitor, but idk if they'll accept it.

Am I cooked..
