Urinary Blockage 4yo Male cat “last resort”

My friends cat has a urinary blockage that was discovered at the emergency vet, took them multiple tries to get the catheter. She was supposed to be able to take him home today if all went well, but she got the it’s an emergency call and rushed over. They said he keeps reblocking and continuing to catheter him would be throwing money down the drain and just prolong suffering. They unblocked one more time and sent her home with some last resort medicine. It was my understanding that there are surgical options that could be a solution but they said that would only be for stones, which I don’t believe to be true. She spoke to three other vets but no one gave a clear answer. Should she try pushing for a surgical option with a different vet or should she just see if this medicine works and if not go through with the euthanasia? I’m sorry for rambling it’s just a stressful time

Update: spoke to my friend and the vet had told her earlier today before giving him the last resort meds and sending him home with her that even with surgery he would reblock. How likely is this and how are they so positive?
