Need some guidance/opinions regarding my father’s prostate cancer case and upcoming surgery.
My father is 60 years old and currently has no symptoms. He was diagnosed with prostate adenocarcinoma (Gleason 4+4 = 8, Grade Group 4). Surgery planned is radical prostatectomy.
We recently got the PSMA PET-CT report and I wanted help understanding how serious/localized this sounds from people who may have gone through similar situations.
Important PET-CT findings/conclusion:
- PSMA avid lesion in left peripheral zone/apex of prostate
- Lesion indenting anterior wall of rectum with loss of fat plane
- No PSMA avid lymph nodal metastases
- No distant metastases seen
- Subtle right 5th rib lesion mentioned but considered likely benign/non-specific due to no abnormal PSMA uptake
Doctors are still proceeding with radical prostatectomy with curative intent.
Questions:
- Does this sound like locally advanced but still operable disease?
- How common is recurrence in cases like this after surgery?
- For people with Gleason 8 + negative PSMA spread, how was long-term recovery/outlook?
- How difficult was urinary recovery after catheter removal?
- Did anyone require radiation/hormone therapy later even after surgery?
Trying to stay realistic but also hopeful since the PET didn’t show obvious distant spread.
Would appreciate honest experiences/advice.
